Non-student Personal Training Sign Up Form

Participant MUST be a SRFC member to sign up for a Personal Trainer.

Each session must be paid for before the session occurs in the administrative office (Room 103) at the SRFC. The administrative office regularly scheduled hours are M-Th 8 a.m. - 8 p.m., F 8am-6pm; Sat. & Sun 11 a.m. - 5 p.m.

A receipt is required to participate in a session.

There will be time spent (approximately 5-10 minutes of your 1 hour session) in your initial session for some consultation.

All paper work must be complete before a session will be scheduled.

Anyone over the age of 40 years old and/or possible high risk factors will be required to obtain a physician's consent.
